Injuries limited his availability during the following two seasons, but his ability remained clear whenever he returned to the field. During October 2022, Carolina traded McCaffrey to San Francisco, placing him within an attacking system designed to maximise intelligent movement, versatility and yards after the catch.
His influence on the 49ers was immediate. In only his second appearance for San Francisco, McCaffrey scored a rushing touchdown, caught a touchdown and threw a touchdown pass during a victory over the Los Angeles Rams. He became one of the few players in NFL history to complete that three-part scoring achievement in the same game.
McCaffrey helped the 49ers reach the NFC Championship Game following the 2022 season, then delivered the finest campaign of his professional career in 2023. He led the NFL with 1,459 rushing yards and finished with 2,023 yards from scrimmage. His 21 regular-season touchdowns also led the league.
During that season, McCaffrey scored in 17 consecutive games when the previous postseason was included, matching the NFL record. He also moved beyond Jerry Rice’s long-standing 49ers mark for consecutive games with a touchdown. His production gave quarterback Brock Purdy a dependable outlet and complemented an offence featuring George Kittle, Deebo Samuel and Brandon Aiyuk.
San Francisco secured the leading seed in the NFC and progressed to the conference championship against Detroit. Trailing 24–7 at half-time, the 49ers produced a memorable comeback to win 34–31. McCaffrey rushed for 90 yards, caught four passes for 42 yards and scored two touchdowns, helping send San Francisco to Super Bowl LVIII.
In the Super Bowl against Kansas City, McCaffrey rushed 22 times for 80 yards and caught eight passes for another 80 yards, including a touchdown. He became the first player in Super Bowl history to record at least 75 rushing and 75 receiving yards in the same game. San Francisco was eventually defeated 25–22 in overtime, but McCaffrey’s all-purpose performance underlined his importance to the team.
His 2023 achievements earned NFL Offensive Player of the Year, First-Team All-Pro recognition and another Pro Bowl selection. He was also voted among the leading players in the league by his fellow professionals, completing a season in which he established himself as the central figure in one of football’s strongest offences.
Achilles and knee problems restricted McCaffrey to four games in 2024, but he responded with an exceptional comeback in 2025. Playing all 17 regular-season games, he carried the ball 311 times for 1,202 yards and ten touchdowns. He also caught 102 passes for 924 yards and seven receiving scores.
His combined total of 2,126 yards from scrimmage ranked among the league’s best, while his 413 touches demonstrated his continued importance to San Francisco. McCaffrey earned First-Team All-Pro and Pro Bowl recognition and was named NFL Comeback Player of the Year. By the end of that season, his career totals stood at 7,589 rushing yards, 626 receptions, 5,390 receiving yards and 98 combined rushing and receiving touchdowns.
McCaffrey also received the 2026 Bart Starr Award, recognising character and leadership alongside excellence on the field. His career has developed through American college football and the NFL rather than a separate senior international tournament structure, so there is no national-team competition record associated with him.
