Itoje was a standout on the 2017 Lions tour of New Zealand and returned for the 2021 series in South Africa. In 2025 he became England captain and then received the greater touring honour: captain of the British & Irish Lions in Australia. The Lions won the first two Tests to secure their first series victory in 12 years and their first in Australia since 1989, finishing 2–1. Itoje became the first Englishman to captain a winning Lions tour since Martin Johnson in 1997.
